When you register, you will be asked to electronically sign this form:

This form must be completed for each soccer player (participant) and, if the player is under 18-years old, must be signed by the player's parent or legal guardian. No player will be allowed to participate in PlaySoccerMiami league or pick-up games without this form, properly executed, and on file.

I, the undersigned, in consideration for my voluntary participation in organized soccer, do hereby willfully acknowledge that my signature below attests to my understanding and agreement that:

My player status will be kept in good standing. I will not compromise myself in such a way as to do harm to PlaySoccerMiami, knowing that players may be dismissed from participation, with possible loss of payment or dues, for violent conduct or unsportsmanlike behavior on or off the field of play.  I agree to pay for any and all damages to any property or indemnities caused by me willfully, negligently, or otherwise.

Soccer is a physical, contact sport that involves the risk of injury.  I assume all risks and hazards associated with my participation in the sport. I am in proper physical condition to participate in league and/or pick-up soccer games and have no illness, disease or existing injury or physical defect that would be aggravated by my participation.  I will inform a PlaySoccerMiami employee if this status changes.  I further acknowledge that this risk may involve loss or damage to me or my property, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of immediate emergency medical care. 

PlaySoccerMiami does not have personal injury insurance that covers my participation.  Therefore, I should have a current, active, personal injury insurance policy in force, which covers my participation.  Under any condition, I am responsible for any and all medical expenses arising from my participation, both in league games, pick-up games and while traveling to and from these events.  I have the right and responsibility to inspect the equipment and facilities prior to events and, if I believe that anything may be unsafe, I will advise a PlaySoccerMiami employee of the condition and may refuse to participate.  Participation assumes consent. 

I authorize my photograph, picture or likeness, and voice to appear in any documentary, promotion (including advertising), television, video, or radio coverage of PlaySoccerMiami, without compensation.

I authorize that an unaltered copy of this form may be generated and given to the officers or directors of any leagues or tournaments in order to allow my participation in their soccer programs, if the form is required and I have requested to participate.

By filing my card in the system, I authorize Play Soccer Miami to charge any due payments to this card.

By requesting the Payment Plan as offered by the league, I accept charges to my credit card to cover the league fees for the duration of the season if applicable. 

Payments received after game day will incur in a dollar service charge. Players should have no outstanding balances to participate in any type of game. 

I hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, the United States Soccer Federation, the State Association, the Club, PlaySoccerMiami league and tournament, their associated directors, administrators, officers, managers, employees, coaches, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors, from any and all liability incurred in the conduct of, and my participation in their soccer programs.  This includes owners, lessors, and lessees of premises, municipalities, government agencies, successors, heirs, and assigns.

I have completely read this document and fully understand its contents. I acknowledge that I have given up substantial rights by accepting this document and that I do so voluntarily.  My signature attests to this on behalf of myself and my executors, personal representatives, administrators, heirs, next-of-kin, successors, and assigns.

Anyone under the age of 18 must have their parent or legal guardian sign a hard copy of the form which can be obtained from a PlaySoccerMiami employee at the field of play.

Season Fees

By choosing the full payment option when registering in a league, I agree to pay the set amount by the end of the first week of the season. Players should have no outstanding balances to participate.

By choosing the Payment Plan option, I agree to pay the initial deposit by the end of the first week of the season and the specific amount for each game the day that it is played. 

PRIVACY POLICY

Your privacy is very important to us. Accordingly, we have developed this Policy in order for you to understand how we collect, use, communicate and disclose and make use of personal information. The following outlines our privacy policy.

• Before or at the time of collecting personal information, we will identify the purposes for which information is being collected.
• We will collect and use of personal information solely with the objective of fulfilling those purposes specified by us and for other compatible purposes, unless we obtain the consent of the individual concerned or as required by law.
• We will only retain personal information as long as necessary for the fulfillment of those purposes.
• We will collect personal information by lawful and fair means and, where appropriate, with the knowledge or consent of the individual concerned.
• Personal data should be relevant to the purposes for which it is to be used, and, to the extent necessary for those purposes, should be accurate, complete, and up-to-date.
• We will protect personal information by reasonable security safeguards against loss or theft, as well as unauthorized access, disclosure, copying, use or modification.
• We will make readily available to customers information about our policies and practices relating to the management of personal information.
